In order to streamline the vaccination process, please print, read and complete the following documentation prior to arriving at your vaccination appointment:
- Vaccine Consent Form
- Notice of Privacy Practice
- Patient Rights and Responsibilities
- Moderna Emergency Use Authorization Fact Sheet
- Pfizer Emergency Use Authorization Fact Sheet
Other documentation required for vaccination:
- CDIB card
- Social Security Card
- Driver’s License
- Insurance Verification (if applicable)
- Proof of employment will be required for teachers under 65
