Community Health Representatives

The scope of service for the Muscogee Nation Community Health Representative (CHR) is to provide health outreach services and health promotion/disease prevention services to Native Americans within their communities. The CHR Program is staffed with five administrative staff, twenty-four Generalist positions and 4 transporter positions.

Diabetes Prevention and Management

The MCN Health Diabetes Prevention and Management Program partners with healthcare providers and community resources to deliver comprehensive care and education for individuals living with diabetes or at risk of developing it. With dedicated teams at five clinics across the Muscogee Reservation, we offer personalized support to help you take control of your health, prevent complications, and lead a healthier, more fulfilling life. Our mission is to empower you with the knowledge and tools needed to manage your diabetes effectively and prevent future health challenges.

About the Service

Employee Health Clinic

The Employee Health Clinic, located near the Okmulgee Medical Center, serves Muscogee  Nation Tribal employees, MCN Health employees and insured dependents with no out-of-pocket expense to the patient.

Forensic Nursing

The Forensic Nursing Program offers specialized, trauma informed care for survivors of sexual violence. Our team is here to support your medical needs with compassion, professionalism, and confidentiality.

About the Service

Mother & Baby Program

The Mother & Baby Program addresses non-medical issues, utilizing the Family Spirit home-visiting model, that put mother and baby at risk, such as addiction, domestic abuse and lack of secure housing and adequate food. In addition to education, in-home or Telehealth visits by a Registered Nurse allow for monitoring and early intervention of common maternal health risks.
