That’s why we address social drivers of health—like housing, food security, education, and transportation—as part of every program we offer.

Our Mission

To empower individuals and communities with the knowledge, tools, and access they need to achieve their best possible health. We do this by advancing health equity, strengthening public health infrastructure, and addressing the social and environmental factors that impact well-being.

Our Public Health Programs Include:

• Community Health Outreach
  Mobile clinics, health screenings, and wellness events designed to meet people where they are.

• Health Education & Prevention
  Vaccination campaigns, testing services, and education to stop the spread of illness.

• Chronic Disease Management
  Programs that support individuals living with diabetes, heart disease, and other long-term conditions.

• Maternal & Child Health
  Prenatal care, parenting resources, and support for healthy early childhood development.

• Nutrition & Healthy Living
  Education and access to healthy foods, physical activity programs, and lifestyle coaching.

2026–2030 Tribal Health Assessment Survey

MCN Health Public Health is asking for community feedback to help develop the 2026–2030 Tribal Health Assessment.

This survey helps us better understand the health needs, priorities, and experiences of our community. Your responses will help guide future public health planning, programs, services, and outreach efforts across MCN Health.

The survey is anonymous and takes approximately 30 minutes to complete.

Survey Incentive Update

While the first 4,000 response gift cards have been claimed, everyone who completes the Tribal Health Assessment Survey will still be automatically entered into a drawing for a $500 thank-you gift card.
